The Kansas Jayhawks commonly use this play with a simple entry to the 5. 1 follows the pass for the handoff looking for 2 cutting off the stagger screen or 5 rolling to the rim. Video clip below.

5 pops off the block to receive the pass from 1.
2 cuts to the opposite corner.
3 & 4 position themselves to set a stagger screen for 2.

1 follows the pass to get a handoff from 5.
2 cuts to the top off the stagger screen from 3 & 4.
1 looks for 2 up top or 5 rolling to the basket after the handoff.
