"Stack 25" is a set that you will likely see in the 2017 NBA Playoffs from the Houston Rockets. They will put James Harden in a high ball screen, with Eric Gordon or Lou Williams setting a back screen. The set is very difficult to guard, due to the spacing, and simplified reads for the point guard.

1 stretches the ball screen, towards the wing

4 rises from the opposite corner
